This form is for use by donors who have already reserved their tax credit through the MyTax Illinois website and are ready to make their contribution. Please complete the Tax Credit Scholarship Program Donor Restriction Form and upload your two-page Contribution Authorization Certificate (CAC) here before making your donation. CACs can also be submitted via email to sgo@bigshouldersfund.org.

Guidelines

Thank you so much for your support of the Illinois Tax Credit Scholarship Program! We strive to provide the highest quality service experience possible for program donors, schools, and benefiting children and families while remaining in compliance with the rules outlined by the state of Illinois Invest in Kids Act.

Please help us streamline contribution processing and ensure timely distribution of scholarship awards by reviewing the following guidelines before making your contribution:

Please alert your credit card company if you will be submitting a payment of significant size. This measure helps ensure your transaction will not be flagged as potentially fraudulent and, therefore, declined. Advise your card issuer of the payment amount, date, and beneficiary of your payment.
Please reserve your credit on the MyTaxIllinois website prior to remitting payment for your contribution. Ensure the Date Certificate Issued for your Contribution Authorization Certificate (CAC) precedes the date of your contribution.
Please confirm Big Shoulders Fund is the Scholarship Granting Organization (SGO) named on your CAC. Big Shoulders will be required to refund your contribution if another organization is the named SGO on the CAC that corresponds to your payment.
If you are not the individual named on the CAC that corresponds to this payment, please do not make a payment on behalf of another individual unless you are spouses residing at the address listed on the CAC.
Individuals making a contribution can restrict or allocate their contribution to benefit multiple eligible schools within the Region indicated on your CAC. However, if designated schools are located in two different regions, you will need to make separate credit reservations on MyTaxIllinois for each region. A list of eligible schools and regions can be found here.
Please ensure the amount of your contribution does not exceed the amount of the credit you have reserved.
Please remit payment for your credit reservation prior to the Contribution Due Date, which is 60 days from the date of the reservation listed on your CAC or the end of the calendar year, whichever comes first. Big Shoulders Fund will be required to refund any contributions submitted after the Due Date and the donor will need to begin the credit reservation and donation processes anew if they wish to qualify for State tax credits.
